   1. Universal City Single Family residential homes may request to have ONE bill adjusted within a TWO-YEAR PERIOD.
   2. Submit this form only AFTER you have received your affected high-water bill and within 60 days.
   3. You MUST ATTACH ITEMIZED COPIES OF YOUR RECEIPT FROM A PLUMBER OR PARTS PURCHASED FOR REPAIR AND PICTURES OF LEAK Adjustments will NOT be considered without proper documentation.
   4. No adjustments under $20.00 will be applied.
